The SQL Server Connection Form is displayed after choosing the "Scriptoria for SQL Server" Tools | Add-ins MS-Access menu item.
After selecting an ODBC Data Source, the name of the SQL Server and ODBC data source description are displayed.
If the ODBC data source was configured to use a trusted connection, the Trusted Connection box is checked by default.
If you are not using a trusted connection, then enter your User ID and Password.
Before you can connect to SQL Server, you must select a database from the combo box. Press OK to establish the connection.
Use the ODBC Data Source Administer found in the Control Panel to register SQL Server ODBC data sources.
